If you would like to request your records from Fredericksburg Eye Care to be forwarded to you or your new optometrist, please leave a voicemail on our record request line (804) 466-4114 or email an employee. Include the following information in your requests:

• Patient’s first and last name
• Patient’s date of birth
• The phone number on file with Fredericksburg Eye Care
• Where you would like to have your records forwarded:
  • Fax number (Preferred option)
  • Mailing address ($10.00 fee for printing/mailing record via USPS)
